To ensure continuity of services and effective clinical outcomes, our staff works with the physician and the patient's family to implement a plan of care. Seniors Choice is seeking a Clinical Case Manager to be responsible for intermittent visits for a geographically assigned caseload of patients. The Case Manager will be responsible for planning, coordinating, providing and documenting care for designated patients. You will provide overall leadership for our nursing program as you evaluate curriculum and teaching methods. Hire, train, and direct staff. Prepare faculty and supply budgets. DON will also pursue active membership in professional nursing organizations and attend relevant conferences and classes. We offer a competitive compensation package including benefits from day one!

Must possess current Indiana RN license. Masters Degree. Minimum 2 years documented teaching experience along with advanced preparation in nursing, education and administration. 5 years in nursing education, AND 5 years of varied nursing experience since graduation from an accredited nursing program.
